Students will need to bring their vaccination records.
Every student enrolling needs to complete the Parent Statement for Food Allergiesand Health History 2012-13 (Spanish form is available). If your child has a food allergy you must complete the Special Dietary Restrictions or Allergy form and have it signed by your physician in order for us to accommodate your child.
For students with special needs parents or guardians must also complete (in addition to the above forms) Health Information Special Needs Detailed. If your student needs any special procedures or medication administration we will need additional forms signed by your child’s physician.

Our objective is to maintain an inviting, and calm atmosphere where students/staff members feel comfortable addressing their needs.
Our goal is for students to be in class as much as possible. We are here to assist with the acute needs of the student body and staff members.
What our clinic does:
- First aid.
- We administer medication with a physician order. There is two forms which need to be completed and turned into the clinic for us to administer medication.
- Provide sanitary needs for female students (a cost shared with the students with them paying $0.25 each item). We do “loan” items out, but may need to come find a student during class for repayment.
- Ensure all students are up to date on their vaccinations.
- Provide some hygiene supplies (soap, deodorant, tooth paste, etc…) to students if their families are unable to provide these items.
- Provide ice packs for day of injuries, dental issues, and post-surgical site(s) during postoperative period with a physician’s note documenting the procedure.
What our clinic doesn't do:
- Anything with piercings or tattoos.
- Provide snacks, water, or other necessities.
- Provide lotion, ointments, or body spray/cologne.
- We do not have any medications for student administration. Students can carry over the counter medications with a parent note (note should include student’s name, medication, dose, frequency, time period, and purpose of the medication).
- Provide crutches, wheelchairs, or medical devices for student’s use.
- Provide treatment for athletic injuries (wraps, medication, and cold/heat therapy).
- Keep students out of class for cramps or undocumented headaches.
- Any diagnosing of medical conditions.
Who our clinic sends home:
- Students who have a fever greater than 100.4., active vomiting, severe acute injury (potentially broken bone, limb, or digit/concussion) or open injury (laceration).
- Diabetics who have a glucose reading > 250 and mod-large ketones in urine that don’t improve with fluid pushing and insulin intervention.
- Asthmatic students that are having an acute attack which is not relieved with their inhaler.

What our clinic reports:
- Suspected abuse.
- Student’s suspected exposure to STDs or HIV.
- Anyone’s exposure to Blood Borne Pathogens.
